    • Complete a board approved radiology seminar/exam within 90 days prior to application for radiology permit
    • Complete a CODA accredited dental assisting program within 12 months prior to permit application & provide sworn statements attesting to previous experience and employment administering radiographs within the last 5 years.
  • Apply to the board for a radiology permit
